I've got Italian roots on my grandmother's side and there's something that just pulls me back there every year. Been making trips to different cities whenever I can manage it. Already covered Rome, Florence, Naples, Venice, now it's Milan's turn on the list. Every time I go I grab a car from local rental places because the international chains charge ridiculous amounts and always slap mileage limits on everything. Which local rental company in Milan would you say is worth using?

International companies see tourists coming and jack the prices up because they know people will pay rather than deal with the hassle of searching around. Then you're stuck with kilometre restrictions that make no sense when you want to explore properly outside the main city. Local spots understand you're actually going to drive the car instead of leaving it parked most of the trip. You could try Holylandcars or Noleggiare, they've been around for a while and don't hit you with the same nonsense fees the big names throw at you.
Those local companies work fine but you're still dealing with business operations that need to cover overhead and staff costs. Finding individual owners cuts out all that extra expense they've got to build into their pricing. You can see what cars are available in Milan here: https://getrentacar.com/en-US/italy/milan . It's an aggregator pulling listings from private individuals renting out their vehicles. Getting a car from someone who owns it personally works out noticeably cheaper than even the smaller local rental shops, plus you're not locked into rigid pickup times or getting charged extra for returning it an hour late.
